A pain elemental is similar to a cacodemon: a large, floating orb-shaped monsters with leathery skin, wide sharp-toothed maw, a pair of horns, and a single large red eye. It has a pair of stubby, three-clawed arms. A pain elemental gathers and consumes lost souls, torturing them in its fiery interior. It is able to release these lost souls to attack anyone it chooses Variant: Rahovart [ edit ] A rahovart is an advanced pain elemental with no maw, but has smaller mouths in place of each claw.
